Description
- Professional basic services of the funeral directors and staff.
- Entering the deceased’s vital information in to the Wisconsin State Vital Records Information System (SVRIS) and obtaining the certified copies of the death certificate (The fee from the Register of Deeds for the certified copies is not included).

- Certified copies of the death certificate will be mailed to the family.

- Obtaining the cremation permit from the county in which the death occurred (The fee for the cremation permit from the County Medical Examiner/Coroner is not included. We will notify you as soon as we are aware of any other fees from the County Medical Examiner/Coroner’s Office.)
- Some counties require that we bring your loved one to their office for a cremation viewing. In these situations, there is an additional $195 fee for this additional mileage. We will notify you as soon as we are aware of this being applicable.
- Entering the deceased’s vital information in to the Wisconsin State Vital Records Information System (SVRIS) and obtaining the certified copies of the death certificate (The fee from the Register of Deeds for the certified copies is not included).
- Transfer of the deceased from the place of death to the funeral home within 25 miles.
- Any additional mileage (distances beyond 25 miles) has a fee of $5.00 per mile one way.
- Our staff is available any time of the day or night.
- Washing and care of the deceased as required per Wisconsin State Statute DHS 136.04.
- Minimum alternative container as required per Wisconsin State Statute 440.78(3).

Cremation at a licensed crematory.
- Cremated remains can be mailed to the family using the United States Postal Service ($150 fee), locally delivered by a courier ($75 fee), or picked up by the family at 2247 South 108th Street West Allis WI 53227 (no fee).
- Temporary Metal Tin to hold cremated remains.
- The prices shown are the now current retail prices and are illustrated for the sole purpose of establishing the amount of insurance required to fund this agreement. At the time the goods and services are provided, the then current retail prices will be charged. The Party responsible for the funeral must pay any difference between the available death benefit and the then current prices, if any.
